Medication Therapy
I meet with my patients and assess their health and wellness goals. My approach is to meet people where they are on their journey to wellness and introduce them to ideas and techniques that will help them to achieve true healing.
Medication Therapy Management (MTM) is a service I provide to help my patients manage their medications in a way that maximizes the effectiveness of treatment while minimizing risks. MTM is helpful for my patients with chronic conditions, those taking multiple medications, or those at risk of medication-related problems.

What To Expect?
1. Medication Review
I will go over your medications, including prescription, over-the-counter (OTC), and herbal products, to ensure they are safe, effective, and appropriate for your condition. I will make sure you understand your medications and why you are taking them.
2. Personal Medication Record (PMR)
I will create a detailed list of all the medications you are taking. This record can be shared with other healthcare providers to ensure coordinated care.
3. Medication Action Plan (MAP)
A plan that outlines specific actions you can take to improve your medication regimen, such as making changes to dosages, timing, or methods of administration.
4. Intervention
If I identify any issues with your medication regimen (like potential drug interactions, side effects, or non-adherence), I will make recommendations and communicate these recommendations with your healthcare providers.
5. Holistic recommendations
I will communicate any holistic wellness recommendations (nutrition counseling, energy balancing, meditation) that will aid in your healing and overall wellness.
6. Follow-up
I will follow up with you and your healthcare providers to assess your progress and ensure continuity of care.
